Job Summary:

You will carry out the duties of a Navigator for Supported Decision-Making clients. You will follow clients leaving the prison system in an effort to prevent plenary Guardianship/Conservatorship.

Qualifications and Requirements:

• Degree in Nursing, Human Services, or other relevant field or two years of experience in a closely related field with similar clients and similar decision-making capacity.
• Willingness to become an Advanced Care Planner to assist in completion of Healthcare Directives.
• Knowledge of public benefits.
• Valid driver's license and DHS/BCA background check.
• Extreme detail attention to communications, accurate and timely documentation, and billings.
• Clear and unbiased thinking and ability to support clients' desires as long as they are legal and do not cause harm.
• Assessment skills.
• Comfort with ambiguity.
