Reports To: Social Service Director
Requirements: Must have a minimum of one year experience in long term care, mental health, or human services. Social Services Designee Certificate preferred.
Job Summary: To participate in a multidisciplinary team approach to provide for the psychosocial welfare, serve as a primary advocate, and meet the psychological and mental health needs of each resident to improve or maintain their highest practicable resident physical, mental, and psychosocial well-being. Ensure Federal, State, local, and facility regulations, guidelines and standards are followed.
Duties for Social Services Assistant, include but are not limited to:
- Monitor patients in order to detect unusual behavior, and report observations to professional staff.
- Provide mentally impaired or emotionally disturbed patients with routine physical, emotional, psychological or rehabilitation care under the direction of nursing and medical staff.
- Record and maintain records of patient condition and activity
- Organize, supervise, and encourage patient participation in social, educational, and recreational activities.
- Interview patients upon admission and record information.
