MAB Community Services is a non-profit human service organization serving adults with developmental disabilities, individuals who are blind or visually impaired, and adolescents with brain injury. MAB is currently building our team with enthusiastic professionals with the desire to rebuild lives and grow professionally.
The TA Coordinator part of the position involves working with individuals in skilled nursing facilities to assist them with a successful transition to the community. Transitional assistance can include any or all of the following:
The TA Coordinator/Case Manager will work with individuals who have transitioned from skilled nursing facilities to living with supports in the community. Case Management can include any or all of the following:
The TA Coordinator/Case Manager should be prepared to transport individuals in their own vehicle on occasion and be prepared to provide occasional personal care if necessary.
Requirements:
-Must have excellent written and oral communication skills, problem solving skills, and basic computer literacy.
-A minimum of two years experience working with adults with disabilities is required.
-A bachelor's degree in a related field is preferred.
-Valid driver's license, clean driving record, and reliable transportation required.
-Updated CPR and First Aid Certifications required, or ability to become certified immediately upon hire.
-This is can be a full-time, benefits eligible position! Schedule is Monday-Friday with flexibility in days and hours based on program need, or part time position with hybrid options
