The celebration of inclusivity and diversity is essential to helping MANNA achieve its mission to involve, educate, and unite all people in the work of ending hunger in Western North Carolina.Project Description: The Healthy Opportunities Pilot (HOP) is an innovative, new approach to addressing the upstream social determinants of health and reducing the need for clinical interventions by using Medicaid dollars to pay for services related to transportation, food, housing, and interpersonal safety. This Medicaid pilot program was developed by the North Carolina Department of Health and Human Services (NCDHHS) to improve the health of Medicaid members and save on costs for the treatment of clinical illnesses by investing in evidence-based interventions. Currently, NCDHHS supports HOP in three areas in North Carolina, including an 18-county region in the western part of the state, with future plans to expand the initiative across the state. As part of this pilot, MANNA FoodBank has established a program and project team to support our partner network with the implementation and management of services to be provided to Medicaid members who are food insecure.
Position Summary: The Project Manager will manage all aspects of project initiation, planning, execution, monitoring and control of the food services for HOP in WNC at MANNA. In collaboration with and as support to Impact Health, the HOP network lead in WNC, and the MANNA Foodbank Network, the Project Manager will provide technical assistance, training, and support to food service partners; assess capacity and gaps across the region and train organizations to close those gaps; and facilitate collaboration with in-network and out-of-network partners to increase project agility as it scales in the region. The position will oversee quality assurance, risk management, data reporting/analysis, and cost management across the project, particularly focusing on direct-to-client services and volunteer management.
Essential Duties and Responsibilities: Design, develop, implement, evaluate and manage HOP services for those who are food insecure. Because the pilot is an innovative approach to Medicaid services that has not been attempted previously, the project will necessarily evolve over the course of its lifetime, and the essential duties and responsibilities of all HOP positions will evolve accordingly.
  • Provide direct-to-client services for the clients that MANNA serves directly as a Human Service Organization (HSO) in HOP. Services may include Healthy Food Boxes, Fruit and Vegetable Prescriptions, Case Management Services, etc.
  • Contribute to the development, evaluation, and revisions of the protocols that outline how food services are implemented both at MANNA and across the HOP Network in WNC.
  • Support the MANNA HOP Management team and the HOP Network Lead in WNC, Impact Health, in training on best practices for food services across the WNC Network.
  • Provide subject matter expertise in case coordination to the HOP project, other MANNA programs and Impact Health.
  • Liaise with HOP care managers and advocate for new HOP related services or adjustments to HOP related services.
  • Track and report on services provided, including oversight of case entries into NCCARE360.
  • Recruit, train and coach volunteers/interns assigned to work with the project team.
  • Handle all sensitive, complex and/or confidential information with care and according to established procedures and ensure that volunteers/interns also handle such information in accordance with procedures.
  • Support the HOP team in complying with requests from the WNC HOP Network Lead, Impact Health.
  • Maintain complete client confidentiality.
  • Assist other MANNA FoodBank activities as needed, including support to strategic planning for the associated program.
  • Perform other duties within the Programs department as assigned.
Qualifications:
  • Education/Experience: A range of education, training, certifications and experience will be considered for this position.
    • This range could include High School diploma or equivalent, combined with three to four years of work experience, and up to a four-year degree with one to two years of work experience.
    • Fields of experience ideally include social work and/or case management, and can include program coordination, food banking, public health, or related field.
  • Bilingual proficiency in English and Spanish preferred.
  • Volunteer coordination/management experience preferred.
  • Customer service, case management, and/or project management experience strongly preferred.
  • High level of skill and comfort with technology preferred; proficiency in Microsoft Office Suite and Outlook required.
  • Must be able to work in a busy office environment, work to deadlines and manage multiple priorities.
  • Attention to detail regarding data entry, analysis and reporting.
  • Outstanding customer relations skills, and eagerness to assist others.
  • Demonstrated cultural diversity and inclusion awareness and practice.
  • Ability to communicate complex information clearly.
  • Strong interpersonal, written and verbal communication skills; ability to work cooperatively and collaboratively with others.
  • Compassionate, non-judgmental, listening skills.

Travel: Moderate travel is required within the service area; occasional overnight travel for conferences or regional meetings.
Employee must provide proof of current, valid driver’s license with clean driving record.
