At Mannor Law Group, we are not your traditional estate planning and elder law firm – we go far beyond providing legal and financial advice. We take a comprehensive, holistic approach to planning, which we call Elder Care Navigation. Our growing firm is seeking an Elder Care Navigator to join our Grand Blanc and Brighton team.

We are a small firm that works collaboratively. We believe in work/life balance, and overtime is very rare. The Elder Care Navigator works closely with other key planning services staff and attorneys to facilitate all aspects of long-term care planning, specializing in assisting clients with a need for long-term care and/or dementia-focused planning.

About the Position

The Elder Care Navigator is a critical part of our team. This role involves working independently, proactively engaging with clients and their families, and ensuring the highest quality of care and support.

Key Responsibilities:

• Assess clients’ independence and ability for self-care, coordinating services with clients’ care providers.
• Counsel clients and families regarding aging issues.
• Provide education, support, and guidance to clients and families on matters relating to health and long-term care.
• Facilitate access to community resources for clients and families.
• Assist clients and families in understanding the long-term care and healthcare system, including public benefits.
• Advocate for clients’ legal rights to quality health care and long-term care.
• Conduct visits/check-ins with elder care navigation clients/families.
• Handle intakes for long-term care matters as needed or assigned.
• Serve as the clients’ primary point of contact within the law firm.
• Engage in community outreach and education through seminars, speeches, workshops, etc.
• Participate in caregivers’ support groups.
• Visit residential care facilities and other providers to build, maintain, and facilitate relationships.
• Serve as a spokesperson for the elder care navigation practice, developing relationships with key personnel in long-term care settings and other referral sources.
• Travel between our three locations: Grand Blanc, Brighton, Rochester, and occasionally to meet with clients or facilities.

Qualifications:

• 2 years of experience in a long-term care setting or care management.
• Strong verbal and written communication skills.
• Ability to adapt quickly to changing workflow needs.
• Capable of making sound decisions and selecting effective courses of action independently.
• Proficiency in MS Word, Excel, Office, and comfortable with learning new software.
• Strong technology skills.
• Degree in social work or human services preferred.
