Responsibilities:

• Conduct comprehensive primary care assessments, diagnosis, and treatment of patients
• Develop and implement patient care plans in collaboration with the interdisciplinary team
• Build strong, lasting relationships with patients to provide personalized care and support
• Educate patients on health maintenance and disease prevention strategies
• Utilize cloud-based electronic health record system for documentation and communication

Qualifications:

• Board Certified Nurse Practitioner (NP, APRN, AGNP, or CNP)
• Minimum of 2 years of experience in a primary care outpatient setting
• Geriatric experience preferred
• Bilingual Spanish speaking skills a plus (not required)
• Passionate about providing high-quality care and contributing to a fast-growing organization
