Mariner Healthcare is Hiring an ADMISSION CARE COORDINATOR / CENTRAL SUPPLY COORDINATOR Near San Jose, CA

SUMMARY

Supports the Admissions Department’s activities in the pre-admission, admission, discharge, and follow-up processes.

Responsible for ordering and tracking supplies.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

as Admission Care Coordinator

·Coordinates resident / patient referral and approval process.

·Conducts tours of facility with potential residents / patients and their family members.

·Tracks pre-admission referral activity daily / weekly.

·Assists the Admissions Manager with arrangements and confirmation of all admissions.Notifies Departments of anticipated admission.

·Assures contracts are completed on a timely basis.

·Ensures specialized equipment is ordered as indicated (reviews contract content for need to obtain prior approval from authorized payer).

·Ensures level of care approval obtained as indicated and / or OBRA screen (PASSAR, MIMR) is completed.

·Completes records and documentation in accordance with company policy and state and federal guidelines.

as CENTRAL SUPPLY COORDINATOR

·Coordinates the ordering of all house stock and chargeable supplies with nursing and other staff

·Establishes PAR with the Director of Nursing.

·Verifies supply order at the time of delivery, inspects for damaged goods, and verifies against purchase orders.

·Accurately enters and maintains data relative to supplies using company software programs.

·Distributes supplies to appropriate locations.

·Obtains data on chargeable supplies used by each resident and transfers the information into the TRIAD system daily.

·Assists with daily and monthly physical inventory.

·Ensures supply room is properly secured and supplies are kept out of reach of residents at all times.

·Cleans equipment in compliance with infection control procedures.

·Coordinates and validate outside rental of equipment.

ENTRY QUALIFICATIONS

·Minimum high school diploma or equivalent required.

·One or more years experience in health care sales / marketing / insurance or commensurate educations preferred.

·Must be familiar with medical terminology.

·Proficient in the use of personal computer.

HOURLY WAGE RATE RANGE: $18.00 TO $22.00 DEPENDING ON EXPERIENCE
