Schwab Rehabilitation Hospital is a freestanding, not-for-profit, 102-licensed bed facility located in Chicago and offers the continuum of care from acute care to inpatient rehab to outpatient therapy for adults and children. Schwab is a proud member of Sinai Chicago and the first fully licensed and accredited rehabilitation hospital in the Midwest.
GENERAL SUMMARY:
Responsible for the coordination of services required for an organized multidisciplinary team approach to patient care. Assures quality cost-efficient care for the rehab patient population. Develop relationships with organizations and individuals that may refer patients to Schwab Rehabilitation Hospital. Provide on-site assessments at referring hospitals to determine patient appropriateness for rehabilitation. Work with referral sources and Schwab's Admitting Department to facilitate timely admissions. Assist Schwab in achieving {budgeted} inpatient census and {budgeted} outpatient units.
ESSENTIAL FUNCTIONS AND DUTIES:
MINIMUM EDUCATION:
MSW, LSW, or RN
MINIMUM WORK EXPERIENCE:
New Grads welcome. One (1) year experienced in social work preferred, preferably in rehabilitation setting and/or discharge planning.
KNOWLEDGE, SKILLS AND ABILITIES:
Excellent written and verbal communication skills.
REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS:
Licensure to practice in the State of Illinois.
