POSITION TITLE: POSC Project Coordinator

REPORTING MANAGER: POSC Program Manager

DEPARTMENT: Family and Children Services 

JOB CLASSIFICATION: Grade Level 

FLSA: Non-exempt

STATUS: Full Time

Location: Office/FSC

POSITION OVERVIEW

Provides ongoing support services and expertise through comprehensive assessment, planning, implementation, and referral, to addresses the needs of families / caregivers by identifying necessary wrap around services to support their treatment and increase their knowledge and skills. Responsible for identifying and coordination of services across various agencies involved with the family. 

enhance parenting skills and increase self-confidence, promote self-sufficiency, and reduce the chances of child abuse and neglect.

ESSENTIAL JOB RESPONSIBILITIES

  • Establishes rapport with family members of the infants and their caregivers.
  • Responsible for establishing wrap around services that address the needs of the infant, caregiver, and family; enhances parenting skills, increases self-confidence, promotes self-sufficiency and reduces the chances of child abuse and neglect.
  • Responsible for initiating all referrals within 45 days.
  • Monitors progress towards identified and stated goals and objectives of the plan.
  • Responsible for implementing and coordinating all services.
  • Maintaining up to date, detailed documentation of all encounters with the family and service providers
  • Maintaining monthly data tracking form to ensure that all entries are entered in a timely manner with all required information.
  • Perform random urine drug screens and breathalyzer test as needed.
  • Conducts regular home visits while the child is in the home to ensure safety

MINIMUM QUALIFICATIONS

  • Bachelor's degree (B. A.) from four-year college or university; or two to four years related experience and/or training; or equivalent combination of education and experience.
  • Department of Family and Children Services experience a plus

REQUIRED SKILLS, CAPABILITIES, KNOWLEDGE

  • Must be computer literate and familiar with all Microsoft Office Products
  • Must have reliable transportation.
  • Must be knowledgeable of community wrap around service providers/
