Licensed Clinical Social Worker - Full-time - New Beginnings/Integrated Care

Summary: Conduct evidence based individual and group therapy sessions and coordinate social service needs for clients in New Beginnings and working in Integrated Care Clinic.

Essential Duties and Responsibilities:

• Interview clients and families to complete bio-psycho-social assessment needs of clients.
• Meet all regulatory standards for program including Medicare.
• Maintain data to complete reports timely and analyze cognitive and affective testing as requested by the Medical Director.
• Function as part of interdisciplinary treatment team and communicate relevant clinical care information
• Participate in Performance Improvement (PI) and program evaluation activities.
• Provide client and family education both during treatment and in preparation for discharge
• Respond to community inquiries of resources available and continuity of care for clients.

Minimum Qualifications/Education: LCPC/LCSW from accredited graduate school or program required with CPR certification. Meet applicable IL certification standards and be familiar with all current relevant regulations for provision of behavior health services and implementation of treatment and discharge planning data.

Licensure: MS/MSW (Masters level)/Counseling education

Shift: Day shift, Monday-Friday
