Organizational Summary

Maternity Care Coalition (MCC) is a community-based nonprofit organization serving Pennsylvania and Delaware with over four decades of commitment to improving maternal and child health and early care and education. MCC’s mission is to improve the health and well-being of pregnant women and parenting families and enhance school readiness for children 0—3. We achieve this through direct service, advocacy, and research, in collaboration with individuals, families, providers, and communities. Position Summary

Responsible for planning, coordinating, and conducting community outreach activities to recruit eligible participants for Healthy Start services, (case management, and health & parenting education groups). Provides culturally responsive case management, care coordination services, and health education to a reduced caseload of pregnant and parenting families. Services are provided during regular business hours as well as evenings and weekends as needed.

Essential Functions

Community Outreach

  • Plan and conduct weekly community outreach activities to recruit pregnant people, mothers, infants, and fathers for case management services in our target zip codes.
  • Work in collaboration with the Community Education Specialist to recruit participants for health and parenting education groups.
  • Coordinate with MCC programs, community-based organizations, and healthcare providers to identify and refer eligible families.
  • Identify new places to conduct outreach activities and maintain collaborations with existing community providers.
  • Attend community meetings, health fairs, and special events to recruit participants.
  • Work in collaboration with the CES Program by attending outreach activities in health centers and other community places to support recruitment and enrollment activities.
  • Complete all necessary documentation to track and report on outreach activities.
  • Assist the HS Program Manager in the maintenance of tracking systems for family recruitment and enrollments. Tracks and monitors all referrals and outreach activities.

Case management

  • Plans and coordinates enrollment of eligible pregnant women and mothers & infants in case management services.
  • Provides case management and care coordination services, including assessment, referral, follow-up, linkages, and monitoring of pregnant women, new parents, and their families.
  • Supports clients in planning for their own health and social service needs by assisting clients in identifying goals.
  • Refers participants needing more intensive follow-up to appropriate agencies.
  • Learns about and makes contacts with community resources needed by clients.
  • Coordinates and connects clients to community resources.
  • Ensures timely scheduling of prenatal, postpartum, well-child, and early intervention appointments.
  • Conducts health promotion activities by providing client education, disseminating educational literature, and distributing disease prevention materials including condoms.
  • Keeps careful records of all contacts made and follows up as necessary. Keeps accurate records of services received by clients. Enter all required data in ETO.
  • Ensures privacy and confidentiality of client records and shares pertinent information only with the client’s approval.
  • As assigned, works with the team to organize client meetings and other special events.
  • Attends orientation and ongoing training sessions on maternal and child health and welfare, patient navigation, child abuse, housing, nutrition, substance abuse, counseling, parenting, and infant and family support issues.
  • Meets with supervisor bi-weekly for reflective supervision and discusses case management and outreach activities.

Experience, Education, and Licensure

  • High School diploma required; Associate’s degree or relevant certification (CHW, CLC, or Doula) preferred; people with lived experience are encouraged to apply.
  • Experience providing community outreach and case management services.
  • Experience with community health or maternal and child health issues.
  • Bilingual (Spanish Speaking), required.
  • Valid driver’s license.

Knowledge, Skills, and Abilities

  • Understanding of MCC’s mission, goals, and objectives and ability to work independently with a high level of energy and contribute as part of a larger team.
  • Knowledge of the concepts and practices associated with community health issues, particularly with pregnant women, prenatal care, child development, parenting, and other services for mothers, young children, and their families
  • Ability to travel in and around the Greater Philadelphia region to provide services as needed
  • Knowledge of and sensitivity to the issues facing the community served; knowledge of community resources
  • Demonstrated leadership skills
  • Good record keeping and organizational skills; ability to manage multiple tasks
  • Excellent oral and written communication skills
  • Excellent computer skills (including data entry and management, word processing, use of the Internet)
  • Ability to work well with individuals and as a team member
  • Knowledge of and ability to link clients to community resources
