Job Responsibilities

As a Licensed Practical Nurse, you will be responsible for coordinating nursing care for patients based on established clinical nursing practice standards. This will involve delivering patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation.

Specific duties for this nursing health care position include:

The Licensed Practical Nurse is responsible for assisting with the coordination and supervision of quality nursing care services for assigned residents that reflect the facility's philosophy, follows established standards for Home Health and/or Residential nursing services, and promotes the individual independence of residents served.

We are looking for Licensed Practical Nurses who combine strong clinical skills with a compassionate, engaged personality and a dedication to ensuring exceptional patient outcomes and a high degree of patient satisfaction. It is also important that you are highly organized and that you demonstrate excellent communication and interpersonal skills, as well as the ability to work well with a diverse population.

Specific qualifications for this LPN HOME HEALTH role include:

Has an active, in good standing, Indiana license as a Licensed Practical Nurse and graduated from an accredited school of nursing. Has strong organizational and time management skills. One-year professional nursing experience providing direct care to residents in long-term care, acute care, restorative care, or a geriatric setting.
