SUMMARY
Assess Level of Care on every patient on admission. Collaborates with the interdisciplinary team members in ensuring patient's needs are met and care is accurately and appropriately documented.
JOB DUTIES
Assess Level of Care on every patient on admission and then on a continuing basis.
Collaborate with the interdisciplinary team members in ensuring patient's needs are met and care is accurately and appropriately documented.
Completes MDS Assessments on every patients as appropriate and dictated by regulatory requirements.
Complete tracking forms for MDS ensuring timely completion and submission, reentry, and discharge.
Contribute to the completion and necessary updating of plan of care on admission and with changes in level of care.
Assists the Case Manager as needed in the coordination and facilitation of the interdisciplinary team conferences.
Ensures the completion of the MDS and Assessment on admission, and with every change in level of care by relevant members of the interdisciplinary team.
Ensures protection of Resident Rights.
Follows established performance standards and performs duties prudently according to nursing policies and procedures.
Maintains current knowledge of regulatory requirements.
Manage timely electronic submission of data to CMS as needed to comply with regulatory requirements.
Monitor data entered into the MDS and plan of care for accuracy and completion.
Monitor specific data that affects CMI and ultimately reimbursement.
Participates in Department of Health Surveys and communicates effectively with all regulatory and governing bodies.
Participates in the Quality Assurance Performance Improvement (QAPI) process, identifies opportunities for improvement, attends QAPI meetings, and assists the QAPI team in the development and implementation of action plans and performance improvement projects related to the MDS and Care Plan processes.
Performs all other duties as assigned.
SPECIFIC JOB DEMANDS
Strength: Light Work - Lifting, Carrying, Pushing, Pulling 20 Lbs. occasionally, frequently up to 10 Lbs., or negligible amount constantly. Can include walking and or standing frequently even though weight is negligible. Can include pushing and or pulling of arm and or leg controls.
Reaching: Frequently - Extending hand(s) or arm(s) in any direction. Handling: Frequently - Seizing, holding, grasping, turning, or otherwise working with hand or hands. Fingers are involved only to the extent that they are an extension of the hand, such as to turn a switch or shift automobile gears.
Fingering: Frequently - Picking, pinching, or otherwise working primarily with fingers rather than with the whole hand or arm as in handling.
Talking: Frequently - Expressing or exchanging ideas by means of the spoken word to impart oral information to clients or to the public and to convey detailed spoken instructions to other workers accurately, loudly, or quickly.
Hearing: Frequently - Perceiving the nature of sounds by ear.
Near Acuity: Frequently - Clarity of vision at 20 inches or less. Accommodation: Frequently - Adjustment of lens of eye to bring an object into sharp focus. This factor is required when doing near point work at varying distances from the eye.
Color Vision: Frequently - Ability to identify and distinguish colors.
MINIMUM EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
Graduate of approved school of nursing, licensed to practice in the state of Pennsylvania.
B.S.N. preferred.
Three years recent Med-Surg nursing experience with a significant geriatric population preferred.
Prefer two years' experience in Rehabilitation Nursing.
BLS required.
WORKING CONDITIONS
Normal resident care environment with little exposure to excessive noise, dust, and temperature changes.
A large part of work time is standing or walking.
Subject to certain distasteful resident care activities and exposure to communicable diseases.
May be scheduled overtime, weekend and/or holiday hours.
May be exposed to potential hazards such as chemical wastes, body wastes, fluids and other possible infectious material.
