JOB SUMMARY

Meals on Wheels of Central Indiana (MOWCI) is seeking a self-driven individual who will actively contribute to achieving team goals and assigned tasks, while maintaining quality relationships with our clients and partners. The Client Services Coordinator – 1 is responsible for promoting a holistic, nutrition-based approach to care by emphasizing the importance of nutrition. The Client Services Coordinator – 1 works closely with clients to promote nutritional wellness. They empower clients to make conscientious, healthy, nutritious choices.

Essential Duties and Responsibilities

Client Services Coordinator - 1 Responsibilities

• Serves as client care support for program and works with internal staff, external clients, community partners and vendors.

• Identifies and confirms the eligibility of new clients while ensuring each referral form is filled out and signed by an appropriate party.

• Contacts all new referrals within 24 to 48 hours.

• Inputs, maintains, and provides up to date client data, in a timely and efficient manner, in MOWCI’s Client Database Management System.

• Corresponds with diverse client base via phone, email, fax or regular mail.

• Attends all required trainings and professional development opportunities.

• Digitally uploads client files while documenting all contact with clients in MOWCI’s Client Database Management System.

• Regularly reviews client files, ensuring they meet documentation standards.

• Performs nutritional and food insecurity assessments with clients at intake, 90-days of service, 180-days of service, and 365-days of service; performs wellness checks with clients, via phone, at 30-days of service and 120-days of service; performs Client Quality Surveys with clients between 250- to 270-days of service.

• Routinely communicates with and updates MOWCI’s Registered Dietitian (RD) on the nutritional status of clients.

• Collaborates with MOWCI’s Pantry Operations Coordinator and/or Client Experience Team to start services for clients.

• Maintains contact with and answers question from staff at partner sites – including vendors and care-sites.

• Identifies areas where finances can be leveraged or reduced.

• Operates office equipment and computer systems and reports malfunctions.

Job Qualification – Knowledge, Skills, and Abilities

Basic:

• Bachelors degree and 4 years of related experience in customer service, consumer support, and/or case management.

• 40 hours per week: 9:00AM – 5:00PM (Monday – Friday)

Preferred:

• Ability to work with diverse populations.

• Ability to interpret information, ideas, and instruction while clearly and accurately communicating data both verbally and in writing.

• Ability to work both independently and within a team environment.

• Intermediate knowledge of Microsoft Office Suite.

• Ability to be flexible by adapting to change and process improvement.

• Ability to work in a fast-paced, ever-changing environment.

• Excellent interpersonal and conflict management skills.

• Demonstrates advanced organizational skills.

• Demonstrates time management skills.

• Demonstrates advance active listening skills.

Salary: $31,000.00 - $38,000.00
