What are the responsibilities and job description for the Care Coordinator position at Med National Staffing Inc.?
Schedule: 8am-5pm MST, HYBRID (field work/home visits/assisted living facilities, shelters, and other settings)
Knowledge/Skills/Abilities
Conducts clinical assessments of members within regulated timeframes to identify those who may benefit from case management, using clinical judgment, health status changes, psychosocial wellness, and assessment triggers.
Develops and implements personalized case management plans in collaboration with members, caregivers, physicians, and other healthcare professionals to address specific needs and goals.
Engages in telephonic, face-to-face, or in-home visits as required to deliver comprehensive support.
Monitors and updates care plans continuously to ensure effectiveness, documenting interventions, tracking goal achievement, and making adjustments as needed.
Maintains an active case load of members for consistent outreach and case management.
Promotes integrated care by coordinating with behavioral health and long-term services, facilitating continuity of care.
May implement wellness programs, including disease management for conditions such as asthma and depression.
Leads interdisciplinary care team meetings and fosters informal collaboration with other team members.
Uses motivational interviewing techniques to educate, support, and motivate members to achieve health-related goals.
Identifies and addresses barriers to care, providing coordination and support to resolve member concerns.
Collaborates with RN case managers and supervisors as needed to enhance care delivery.
Provides consultation and resources within Behavioral Health and Social Science disciplines to support team efforts.
Up to 40% travel may be required, based on case complexity, regulatory requirements, and program location.
Required Qualifications
Years of Experience: 1-2 years
Licensure/Education: Bachelor's Degree or minimum of 1 year in Care Coordination with at least one year of experience in a healthcare setting.
Preferred Qualifications
Experience: 1-2 years in case management, disease management, managed care, or medical or behavioral health settings.
License/Certification/Association: Preference for one or more of the following credentials:
- Licensed Clinical Social Worker (LCSW)
- Advanced Practice Social Worker (APSW)
- Certified Case Manager (CCM)
- Certified in Health Education and Promotion (CHEP)
- Licensed Professional Counselor (LPC/LPCC)
- Respiratory Therapist
- Licensed Marriage and Family Therapist (LMFT)
