Position Description
Administrative Support/Community Health Worker (CHW) will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide member education. Ability to work in a team-based structure engaging with members, peers, community leaders in a professional manner.
This position is an office-based position; Not telecommute.
Primary Responsibilities:
o Create a positive experience and relationship with the member.
o Greeting members as they arrive to our center.
o Answer calls within the center and market as appropriate
o Assist with community initiatives as outlined by the health plan.
o Experience in professional oral communication and written communication in an electronic based system
o Experience with computer and windows PC applications to include Microsoft- Excel, Microsoft word (basic navigation, functioning and formatting), Outlook, Teams, PowerPoint etc.
o Proactively engage the member to manage their own health and healthcare benefits.
o Support the member to maintain their health and wellbeing as well as assisting member in continuing to improve their health to stay out of the ER and hospital.
o Assist member in engaging with mental health and substance use treatment (as needed)
o Provide member education on community resources and benefits.
o Assist the member in accessing transportation services to ensure they attended.
scheduled medical appointments.
o Conduct post discharge activities from hospital and support connection to social services
o Collaborate/Partner with other members of the members care team (community, providers,
internal staff)
o Continue advancing knowledge and learning of community cultures and values.
o Other duties deemed necessary by UHC management staff.

Required Qualifications:
o High School Diploma /GED (or higher)
o Ability to travel locally, up to 100 miles round trip.
o Knowledgeable of the resources available in the community
o Must be a resident of the state.
o Must have your own means of transportation with automobile insurance in place.
o Must be available to work Monday through Friday 8 AM to 5 PM
o Knowledge of the culture and values of the community
Preferred Qualifications:
o Previous Case Management experience
o Certified Nursing Assistant or Certified Medical Assistant
o Ability to travel locally, up to 100 miles round trip and up to 75% of work time.
o Experience working in Managed Care
Knowledge of Medicaid and/or Medicare population
Job Types: Full-time, Contract
Pay: $18.49 - $22.27 per hour
Expected hours: 30 – 32 per week
