Your Mission
  • Patient Care: Assess health care needs and develop personalized care plans to meet interdisciplinary goals.
  • Responsive Actions: Act swiftly on significant changes in patients' health conditions.
  • Team Collaboration: Carry out care plans by performing and delegating tasks effectively.
  • Data Management: Collect and document essential data in electronic medical records.
  • Nursing Assessments: Complete thorough assessments within required time frames, following policy and procedures.
  • Proactive Care: Identify and address predictable needs of patients/residents.
  • Holistic Support: Meet the emotional and spiritual needs of patients/residents and their families.
  • Efficient Task Management: Prioritize and complete tasks within your shift.
  • Care Plan Updates: Keep patient/resident care plans current.
  • Communication: Notify physicians of any changes in condition.
  • Safety and Reporting: Report incidents of verbal or physical abuse immediately to the Nurse Manager or designated off-shift nursing leadership.
Key Responsibilities
  • Perform comprehensive head-to-toe assessments.
  • Insert and manage IV lines, including peripheral and central lines, and PICC lines.
  • Administer medications, including IV medications.
  • Coordinate and participate in patient admissions and discharges.
  • Measure vital signs and conduct hemodynamic monitoring, including pain assessment and pulse oximetry.
  • Perform wound care, including sterile and non-sterile dressing changes, and obtain wound cultures.
  • Obtain and deliver lab specimens.
  • Perform tracheostomy care and non-acute/acute ventilator care as needed.
  • Conduct suctioning, including oral, nasogastric, and tracheostomy suctioning.
  • Perform deep suctioning and obtain specimens.
  • Administer tube feedings.
  • Assist with urinary and bowel elimination, including catheters, colostomies, and digital stimulation.
  • Perform enemas and other supportive measures as needed.
  • Educate patients/residents and their families about health conditions, diagnoses, and medication administration.
  • Respond to and assist in emergency situations.
  • Provide clinical oversight to Licensed Practical Nurses, Nursing Assistants, and Health Technicians.
