Medical Temporaries, Inc. is currently seeking several experienced ***Case Manager Housing in the Charlottesville/Fredericksburg, VA area (22903)*** .
Minimum of 1 (one) year of experience required.
This is a FULL TIME/TEMP TO HIRE (COULD BE A DIRECT HIRE OPPORTUNITY FOR THE RIGHT INDIVIDUALS) position requiring availability between the hours of 8AM-4:00pm OR 9AM - 5:00PM, Monday through Friday, located in Charlottesville/Fredericksburg, VA (22903).
Responsibilities :
A Day in the life of a Case Manager Housing :
As a Case Manager Housing (Outreach Case Manager) you will work with the Team Lead to serve your clients: low income and disabled adults who may be experiencing homelessness and repeated incarcerations. Provide case management expertise such as assessments, treatment planning, supportive counseling, linking to community resources, monitoring progress, collaboration with other providers, and care coordination. The Supportive Services Specialist will also assist in outreach/engagement activities with clients, families, community providers, landlords and medical personnel. Advocate for clients in court, testify on behalf of the client and/or services, and collaborate with law enforcement, public defenders, DSS and departments of corrections. Maintain client contact and files, treatment plans, progress notes, and discharge summaries.
Requirements for the Case Manager Housing (Outreach Case Management) :
Required: 1 years of experience working in the field of direct services with adults with low-incomes, physical disabilities, behavioral health and developmental disorders, and/or history of homelessness and incarcerations
Required: Bachelor's Degree in Human Services with 1 years of experience as detailed above
Required: or Bachelor's Degree in another field outside of Human Services with 5 years of experience in the field
Required: 3 years of experience working in the field of direct services with the homeless population and an Associate's degree in human services.
Required: Knowledge of mental illness, substance abuse disorders, community resources, especially entitlement and housing resources
Required: Ability to pass Background Check and Drug Screen (9 panel)
Required: Must be reliable and a quick learner and have ability to multi-task with accuracy in a very busy environment
Required: Excellent written and verbal communication skills
Required: CPR certification or CPR certifiable within 30 days
Required: Valid driver's license and acceptable driving record through DMV (travel about 10%)
Preferred Qualifications for the Case Manager Housing (Outreach Case Management) :
Preferred: SOAR certification and expertise
Preferred: CPR/First Aid
Preferred: Prior experience in a human services environment supported by grants; knowledge of electronic health records/data management and Medicaid billing
Benefits:
$20.19-$21.63 an hour (depending on experience level)
