Utah, is seeking a skilled and dedicated Registered Nurse (RN) to

join our team on an on-call basis.

In this role, you will play a vital role in providing exceptional

care to patients in their homes, ensuring their comfort and

dignity during this critical time.

Responsibilities:

• Provide comprehensive end-of-life care to patients in their

homes, following established care plans and physician

orders.

• Conduct thorough assessments and document patient status

accurately using OASIS and Matrix EMR systems.

• Administer medications and treatments as prescribed.
• Educate and support patients and their families regarding end-

of-life care options and emotional coping.

• Collaborate with physicians, social workers, and other

healthcare professionals to ensure a coordinated care

approach.

• Maintain a clean and safe environment for patients.
• Document all care activities and observations clearly and

concisely.

• Uphold high standards of professionalism and compassion in all

interactions.

Qualifications:

• Active Utah RN license in good standing.
• Minimum of 2 years of experience in home health or hospice

care.

• Strong clinical skills and experience with end-of-life care.
• Proficient in OASIS and Matrix EMR documentation systems.
• Excellent communication, interpersonal, and assessment skills.
• Compassionate and empathetic nature with a commitment to

patient-centered care.

• Ability to work independently and as part of a team in a fast-

paced environment.

• Reliable transportation and the ability to travel to patient homes

within Salt Lake County.
