We are currently hiring a Fully Remote Care Manager! The candidate must hold an active License of Social Work in NY (LMSW/LCSW) and have 2 years of Health Plan (Insurance Case Management experience) to be considered.

Overview:

We are looking for a self-motivated and passionate Social Worker Care Manager, who will provide telephonic assessments of member's status and needs. This position works with our patients to develop, implement, and monitor their care plan and ensure continuity of care while monitoring long term care needs.

Specifics:

Hours: Mon-Fri FT 9am-5pm (no part time options)

Location: Remote (plan based out of NY)

Salary: $75,000 - $85,000

Duties:

• Providing all aspects of care management such as planning, coordination, assessment of health, environmental, and psychosocial status of the members on an ongoing basis.
• Identify health, environmental, and psychosocial risks for assigned members and recommend appropriate interventions.
• Provide an on-going assessment of the effectiveness of the Member's plan of care.
• Make requests for authorization to the Care Management Supervisor for new services and changes to the plan of care.
• Assist, as needed, in the semi-annual reassessments using the New York State designated assessment tool and other plan defined assessment tools.

Requirements:

• Active Social Work License in NY (LMSW/LCSW)
• 2 years of Health Plan (Insurance Case Management experience) to be considered
• MLTC experience is required
