Summary: Seeking a Licensed Professional Counselor or LCSW

- Provide coping skills, counseling services, and appropriate support for patients during in-office or telehealth sessions.

- Document all communications with patients clearly and accurately with AthenaHealth electronic records system

- Work with patients with multiple comorbidities including chronic health conditions, mental health concerns, Social needs, and obstacles in their daily lives in an integrated care setting.

- Provide continuous follow-up counseling sessions with patients to adhere to treatment plans and assess ongoing patient needs.

- Demonstrate knowledge of differences between populations of differing ages, cultures, and home life situations – including psychological, physiological, and developmental differences between each group.

- Assist patients in setting goals for their treatment plans
- Ensure all patient information is kept confidential
- Work alongside medical providers to develop and evaluate ongoing patient plans
- Demonstrate commitment to company values

Job Types: Full-time, Part-time

Pay: $55,000.00 - $85,000.00 per year
