Job Summary:
The Minimum Data Set (MDS) Coordinator does market of Pipeline Health’s Sub-Acute Units to acute and long-term acute facilities. Performs physical and financial assessments for patients referred from acute and long term acute. The MDS Coordinator completes and transmits MDS & Treatment Authorization Request (TAR). This position will report directly to the Nurse Director of the assigned facility/department. Actively and consistently contributes to department operations and communications, behaves in a manner consistent with the mission, vision, and values of Pipeline Health, upholding standards of AIDET (Acknowledge, Introduce, Duration, Explanation, Thank you) patient communication.
