Position Title: Dementia Navigator

Memory Care Home Solutions (MCHS) is a 501(c)(3) nonprofit organization that exists to improve dignity and quality of life for people living with dementia and their families by transforming evidence-based interventions into accessible healthcare solutions. MCHS has developed a package of quality, evidence-based services to equip family care partners to effectively manage dementia progression at home and sustain meaningful relationships within the family.
This organization was founded in response to a gap in individualized services for families affected by dementia and has attracted staff, board members, donors, and volunteers who identify personally with the mission.

Position Overview
The Dementia Navigator will provide evidence-based dementia intervention and support for people living with dementia and caregiving families in the community, via telehealth, and at Memory Care Home Solutions headquarters and unique caregiver training environment. The Dementia Navigator assists clients in addressing and prioritizing mental health concerns specific to dementia and the caregiving role, connecting clients with resources in the community, and providing ongoing support throughout dementia progression. This employee will also provide related educational outreach to healthcare professionals and community groups. The Dementia Navigator reports to the Director of Programs and is 40 hours / week full-time position.
