Program Coordinator

35 hours per week / $26.00 - $30.00 per hour

Supervision Responsibilities: Under the guidance and directive of the Adult Team Services Program Manager, will assist with the organization and direction of the Personal Services Coordinators and other assigned staff of the Adult Integrated Service Team. Provides leadership responsibilities for team members during program or service activities and provide secondary on-call services based on the psychosocial rehabilitation model of treatment and services.

Summary: Under the general supervision of the Program Manager the Program Coordinator will assist with the work of multi-disciplinary team providing services to program members. The goal of the entire team is to assist people with mental illness to live as independently as possible in their chosen community. The Program Coordinator works under the direction and guidance of the Program Manager as a trainee learning to building the leadership skills needed to work cooperatively with the service teams; to plan, coordinate and facilitate the smooth and efficient delivery of all Personal Services Coordinators and allied services/programs.

Essential Duties and Responsibilities: This is not an exhaustive task list, and other duties may be assigned as deemed appropriate or necessary by the Program Manager or Program Director. QUALIFICATIONS:

  • Oversees and ensures programs monthly, quarterly, and yearly data reporting. With accurate and timely submission and documentation.
  • Assists in coordination of outreach, enrollments, and transitions between teams and disenrollment’s of program members.
  • Assists and supports program documentation standards of staff in accordance with MHALA, County Contracted Provider, and funding source standards and requirements, and documents in required program including 24-hour turnaround documentation.
  • Actively provides support to individuals on the team as directed and requested by the Program Director or Program Manager and supports publicly all decision made by the management team.
  • Supports staff in all activities that link members and other clients to meet member/client goals and needs.
  • Participates as a Program Coordinator to the team, staff and community meetings.
  • Assists with the leadership of the day-to-day operations of the department as needed.
  • Participates as an active member for quality improvement, safety committee, domains or other leadership teams as needed or assigned.
  • Participates in the agency’s community life as appropriate.
  • Provides timely information and feedback to the Adult Integrated Service Team in regards to team, program or member issues.
  • Provides emergency coverage as needed.
  • Maintains a member list of 5 members and meets a billing goal of 50 hours per month in order to build and maintain the skills necessary to mentor and support the Adult Integrated Team PSC’s in accordance with the expectations outlined in the Full Service Partnership guideline.
  • Attend and participate in staff meetings and other aspects of the day-to-day operations and community life of the agency.
  • Work cooperatively with other staff to assure the smooth and seamless delivery of supportive services to the agency and the members and their families it serves.
  • Assist and support program documentation standards of staff in accordance with MHALA, county contracted FSP provider, and funding source standards, requirements, and documents in requires program, including 24-hour turnaround documentation to ensure all required data collection documents are submitted on or before due date.
  • Collaborates with members to establish a working budget for sustainability of housing and basic needs including all expenses and income sources of the household.
  • Mediates and advocates on behalf of the member to help him/her obtain and keep housing, health care benefits, entitlements, etc.
  • Engage in collateral duties as directed by Program Manager and Program Director as needed.
  • Provides timely information and feedback to Program Manager and Program Director with regard to emergent staff, team or member issues and work with executive management to provide intervention to redirect staff and/or programs when objectives are not being met.
  • Visit members in their place of residence on an as needed basis in order to help them live successfully and prevent them from becoming homeless.
  • Transports members to appointments and on errands when appropriate.
  • Participate as an active member of the site management team, quality improvement, and other designated leadership team as needed and assigned.
  • Monitor completion of recovery plans with each member that identifies his/her personal needs and goals.
  • Read perform researches, attend classes, workshops, Relias trainings or seminars and participate in other professional development activities in order to maintain and enhance personal and professional skills.
  • Mediate and advocate on behalf of the member to help him/her obtain and keep housing, health care benefits and entitlements.
  • Completes all documentation (some specifics: transitions, discharges, 3 months, KEC’s, determinants, MORS and monthly reports, individual service plans) and maintains all necessary paperwork documentation as required for program compliance and accountability in accordance with MHA standards and funding source requirements

EDUCATION & EXPERIENCE MATRIX --one of the following combinations is required:

High School Diploma or GED Minimum of 5 years’ experience providing psychosocial rehabilitation related services to disabled population plus experience in a supervisory/leadership role.

or

AA degree in a human service field Minimum 4 years experience providing employment-related services or sales.

or

Bachelors degree in a specific rehabilitation related field or marketing (e.g. rehabilitation counseling) Minimum 4 years experience providing employment-related services to a disabled population or sales.

OTHER POSITION REQUIREMENTS:

  • Demonstrates ability to provide leadership to diverse staff who possess a wide range of skills, abilities and professional competencies.
  • Must have good basic English and writing skills, good computer skills and be highly organized.
  • Must be detail oriented, and able to prioritize and juggle multiple projects without losing productivity.
  • Knowledge of community resources.
  • Must function well in a team-oriented approach to member services.
  • Must be organized and efficient in carrying out day to day duties.
  • Must take initiative and be proactive in approach to getting the job done with little or no direction.
  • Must be able to work with various personalities and handle aspects relating to team members with dignity and tactfulness.
  • Knowledge of Microsoft Word, Excel, PowerPoint and Outlook required.
  • Driving with current class C driver’s license, proof of valid automobile insurance, and the ability to be insured by MHALA liability insurance are conditions of employment.
  • TB clearance and re-testing every year are conditions of continued employment.
  • Flexible work schedule to include some evenings, weekends and holidays.
  • COVID vaccination is required for this position. Accommodations will be considered with proof of a legal exemption.
