In this role, you will:

• Build relationships with chronically homeless individuals, focusing on those recently incarcerated
• Operate from a virtual mobile office, primarily in downtown areas, encampments, and homeless gathering spots
• Collaborate continuously with family, shelters, outreach managers, and law enforcement
• Ensure individuals are on housing priority lists and receive necessary services
• Facilitate multi-organizational efforts for collective impact interventions
• Integrate with disaster response entities to enhance recovery efforts
• Provide "warm handoff" to other care systems for ongoing stability

The ideal candidate will:

• Be adaptable and available for work hours between 7:30am-6:00pm, M-F, with schedules varying slightly based on weekly events.
• Possess a minimum of a HS Diploma or GED
• Be comfortable working with vulnerable populations
• Demonstrate proficiency in using computers, phones, and office software
• Exhibit excellent communication, interpersonal, and organizational skills
• Manage a high volume of calls while handling front desk duties
