We are seeking a dedicated and detail-oriented Assessor to join our team. The Assessor will be responsible for conducting comprehensive diagnostic assessments, developing individualized treatment plans, and completing DLA-20 and GAINS assessments.
Key Responsibilities:
- Diagnostic Assessments:
- Conduct thorough diagnostic assessments to determine the needs and appropriate services for clients.
- Utilize standardized assessment tools and clinical judgment to gather relevant information.
- Treatment Planning:
- Develop individualized treatment plans based on assessment findings and client goals.
- Collaborate with clients, families, and other professionals to ensure comprehensive and effective plans.
- DLA-20 and GAINS Assessments:
- Administer and score the DLA-20 (Daily Living Activities) assessment to evaluate clients' functioning in various areas of daily life.
- Conduct the GAINS (Global Appraisal of Individual Needs) assessment to identify clients' strengths and areas of need.
- Documentation:
- Ensure all assessments and treatment plans are accurately documented within 48 hours of completion.
- Maintain thorough and organized records in accordance with agency policies and regulatory standards.
- Interviews and Data Collection:
- Conduct interviews with clients in person and over the phone to gather comprehensive assessment data.
- Utilize motivational interviewing and other evidence-based techniques to engage clients effectively.
- Collaboration and Communication:
- Work closely with other team members and send referrals, including therapists, housing, and medical staff, to provide coordinated care.
- Communicate assessment findings and treatment recommendations clearly and effectively to clients and other stakeholders.
Qualifications:
- Education:
- Bachelor's or Master's degree in Social Work, Psychology, Counseling, or a related field.
- Licensure/Certification:
- LICSW, LPC, LMFT, LGSW, LGPC, NP
- Experience:
- Previous experience in conducting diagnostic assessments and developing treatment plans.
- Familiarity with DLA-20 and GAINS assessments is highly desirable.
- Skills:
- Strong clinical assessment and diagnostic skills.
- Excellent written and verbal communication abilities.
- Proficiency in electronic health record (EHR) systems.
- Ability to work independently and as part of a multidisciplinary team.
- High level of organization and attention to detail.

Pay: $50.00 - $75.00 per hour
