About The JobThe Social Services Technician collaborates with other members of the multidisciplinary team in caring for the psychosocial, financial, emotional, environmental factors as they relate to patient health needs; acts in concert with the social services, nursing, medical, and other support staff in providing assistance and supportive measures to patients and families for care coordination and care management – specifically focused on connecting patients with community resources and helping patients to be successful in removing socio-economical barriers; participates in ongoing continuous quality improvement processes to improve patient care.
EducationWhat we are looking for:Bachelor of Science or Arts degree with a major in social services, psychology, sociology, human relations, health related field required.
ExperiencePrevious experience working in connecting individuals to community resources is required.
Special Skills, Competencies, And AptitudesAbility to work on multiple projects at one time and flexibility to meet department needs.
Self-motivated, detail oriented individual able to work with minimum supervision.
Completes The Following Mandatory Courses (check All That Apply)Must provide proof of Basic Life Support (BLS Healthcare Provider, American Heart Association; or American Red Cross, Professional Rescuer-LIVE class only) via current card prior to employment or within 60 days of hire and maintains current status.
Completes Mandatory Reporting: Child and Dependent Adult Abuse requirement within 6 months of hire and renews this requirement every 5 years.
