Who We Are:

Merrimack Valley Assistance Program (MVAP) is a long-established HIV/AIDS Service Organization founded in 1995. As a 501(c)(3) nonprofit and state-licensed case management agency, MVAP provides essential services including case management, supportive services, housing assistance, and referrals to individuals living with HIV/AIDS.

Our Mission:

To provide, or help gain access to, essential services such as safe and affordable housing, adequate food and nutrition supplements, proper medical and dental care, and behavioral health services, as well as to provide education to the community-at-large.

Our Vision:

· Honor the inherent dignity and strength of people living with HIV/AIDS.

· Support and assist persons infected and affected by HIV/AIDS in maintaining a high quality of life through direct assistance, advocacy, and education.

· Strive to prevent new HIV infections and promote safer practices through education/prevention activities for local and regional communities.

· Counteract myths and stereotypes about HIV/AIDS.

· Increase and diversify the network of HIV/AIDS-related service providers in the communities that we serve.

· Play an active role in affecting local, state, and national policies on civil rights, discrimination, HIV/AIDS prevention, education, research, and direct care.

Who We Are Looking For:

MVAP is hiring a Case Manager located in our Manchester office. As a Case Manager, you will manage your own client caseload of approximately 40 clients, being their direct point of contact. You will ensure clients remain engaged and enrolled with us and our programs. You will assist them to obtain services such as access to medical appointments including a PCP and/or HIV Specialist, safe and stable housing, food and nutritional services, harm reduction materials, and more. You will provide them with psychosocial case management, harm reduction counseling, supportive services, assistance with applications and health care navigation, community referrals, and enroll them into our programs such as:

NH CARE Program - The Ryan White NH CARE Program pays for continued health insurance and cost-sharing support toward outpatient medical services for HIV health care. This includes related office visits, medications, labs and diagnostic testing.

HOPWA Program - The Housing Opportunities for Persons with AIDS (HOPWA) Program is the only Federal program dedicated to providing housing assistance to people living with HIV/AIDS. This includes emergency rental assistance, a long-term voucher program, and assistance with securing stable housing such as payment for application fees or security deposits.

Compensation: $21-$24 hourly, based on education/experience.

Schedule: Monday-Thursday, 8am-5pm & Friday 8am-11:30am (37.5 hours per week).

Supervised by: Executive Director

Note: All case manager positions are funded, at least in part, by government grants and contracts. While renewal is anticipated, it is not guaranteed. However, MVAP has successfully secured continued funding since 1998.

Responsibilities:

· Be the main point of contact and maintain consistent communication for a caseload of approximately 40 clients.

· Evaluate and document all interactions with clients and determine program eligibility according to program criteria.

· Provide risk reduction counseling in relation to sexual health, behavioral health, and substance use.

· Assess the needs of clients, with special attention to basic life needs such as food, safe and stable housing, and appropriate health care.

· Develop goals (or Individual Care Plans) with clients, identifying any obstacles and strategies on how to achieve them.

· Provide information and assist with applications for clients, within MVAP and other local agencies, about housing options, community services and/or government agencies and their respective programs, services, or benefits for which they may be eligible.

· Assist clients in making inquiries about and/or applying for adequate housing, applicable programs via the State of NH such as Medicaid, SNAP, etc. and other services or benefits.

· Work independently and collaboratively with the staff of MVAP and various other agencies to establish a coherent and coordinated delivery of services to clients.

· Perform such other duties in the service of clients as may be requested by the Executive Director

Requirements:

· A combination of the following education and/or work experience in an applicable field such as the medical or human services field (nursing, public health, social work, case management, psychology, housing, counseling, health education, etc.) is required: Bachelor's Degree with two (2) years of experience, or Associate's degree with three (3) years of experience, or high school diploma/GED with five (5) years of experience;

· Knowledge of sexual health topics including HIV/AIDS, Hepatitis C, sexually transmitted infections (STIs), PrEP, PEP, viral suppression, modes of transmission, and preventative strategies (strongly preferred).

· Experience navigating a caseload and knowledge of services clients could benefit from and be eligible to apply for (strongly preferred).

· Experience applying and filling out documentation for government programs such as Medicaid, Medicare, SNAP, SSI/SSDI, etc. (strongly preferred).

· Ability to work in a culturally diverse environment, with clients and MVAP staff, where ALL individuals are accepted and welcomed. These communities include bilingual/trilingual language speakers, LGBTQ , Refugee, Immigrant, Undocumented, BIPOC, etc. (required)

· Knowledge of community programs including food pantries, health care facilities, substance use services, and support groups and programs. (strongly preferred)

· Ability to talk with clients one-on-one about sensitive and/or private information including demographics, medical history, current health status, income, and life experiences. (required)

· Excellent verbal and written communication skills, as well as being organized and managing a scheduling clients and meetings (required)

· Ability to work collaboratively with staff members from various agencies, but also independently with each of your clients. (required)

· Knowledge of housing options in the community such as Public Housing Authority (PHA) vouchers, shelters and warming stations, and other means to support actively unhoused clients. (strongly preferred)

· Experience working with various computer programs and applications, to carry out day-to-day tasks and duties, with Microsoft Word, Excel and Outlook, and the ability to learn new programs, applications, and databases, utilized by MVAP. (required)

· Possession of a valid driver's license, reliable transportation, and current automobile 100/300 liability coverage insurance coverage (required). Proof of both will be provided to one's supervisor annually.
