Job Summary and Qualifications

The Social Work Team Leader LCSW will evaluate the psychosocial needs of patients and family support systems and will coordinate appropriate treatment plans for identified patient populations. The Social Work Team Leader acts as a liaison between the facility and resources external to the organization. He/she will facilitate implementing timely treatment plans and facilitate follow-up to anticipated post-acute interventions identified in the plan of care. The Social Work Team Leader will provide crisis intervention and support. The Social Work Team Leader will assist the Case Manager in facilitating patient movement across the continuum of care and will identify and track barriers to patient throughput. In addition to these responsibilities, the Social Work Team Leader will be responsible for overseeing the duties of other Transplant Social Workers and work closely with his/her Manager in providing leadership, direction, and policy development. Additional duties may include; delegating Social Worker assignments, preparing schedules, and monitoring admissions and discharges.

What qualifications you will need:

• Master’s Degree in Social Work
• Two or more years of social work experience required.
• Transplant experience preferred
• Required: Social Work License to practice in the State of Texas (LCSW)
