Home Health Physical Therapist - $10,000 Bonus OpportunityOverview: Our Award-Winning Client, a national leader in healthcare services, is seeking a dedicated Physical Therapist to join their Home Health team. This is a full-time, onsite position with paid relocation. As a member of our client's esteemed team, you will play a pivotal role in providing exceptional Physical Therapy services to patients in the comfort of their homes.
Key Responsibilities
Key Responsibilities
- Conduct comprehensive Physical Therapy assessments and design individualized treatment plans.
- Document patient progress and communicate any changes in condition effectively.
- Educate patients and caregivers on therapy techniques and equipment usage.
- Supervise support staff, including physical therapy assistants and certified nursing assistants.
- Generate reports and actively participate in staff development activities.
- Master's degree or higher in Physical Therapy.
- Minimum 1 year of experience in a Healthcare Setting.
- Valid Physical Therapist license in the state of Illinois (IL).
- Valid Regular Driver License (DL).
- PT license number.
- Proficiency in OASIS & HCHB preferred.
- Full-time, 40 hours per week.
- Monday to Friday; day shift with start time depending on patient appointments; no appointments scheduled past 5:00 pm.
- 1 weekend shift (Saturday & Sunday) every 6 weeks with scheduled patients.
- 2 holidays per year.
- Local travel to residential homes.
- Chicago, Evanston, and surrounding areas.
- Note: This team does not attend to patients on the south side of Chicago. The NSH south border extends only to North Avenue, encompassing the North Side of Chicago. Territory also includes the East side of Chicago along the lake.
- $10,000 First-Year Service (FYS) bonus.
