QualificationsRequired:
- Master's degree or higher
- Minimum of 1 year of experience in a healthcare setting
- Valid Physical Therapist license in the state of Illinois (IL)
- Valid Regular Driver License (DL)
- No previous application to or employment with Residential Home Health & Hospice within the past 6 months
- Provision of PT license number
Key Responsibilities
- Conducting Physical Therapy assessments and formulating treatment programs
- Documenting and communicating patient responses to treatment and changes in condition
- Educating patients and caregivers on therapy protocols and device utilization
- Supervising physical therapy assistants and, where applicable, certified nursing assistants
- Compiling reports and engaging in staff and Agency developmental initiatives
- Monday to Friday; 8:00 am/9:00 am - 5:00 pm
- One weekend per month (Saturday & Sunday; daytime shifts with scheduled patients)
- Two holidays annually (one summer, one winter); no on-call duties
- Local travel to residential homes
