Candidate Details: We are looking for candidates who are dedicated to fostering therapeutic success in home settings, have a solid grounding in physical therapy practices specific to adult care, and who are adept at managing both individual caseloads and contributing to team objectives.

QualificationsRequired:
- A Master’s degree or higher in Physical Therapy.
- Valid Physical Therapist license in Illinois.
- A minimum of one year experience within a healthcare environment.
- A valid regular Driver’s License.
- Kindly include your PT license number with your application.
- Develop and implement comprehensive treatment programs following thorough assessments of patient conditions.
- Instruct both patients and caregivers on the correct use of therapeutic devices and prescribed therapy programs.
- Accurately document treatment reactions and any changes in the patient's condition.
- Actively participate in professional development activities within the staff and agency.
- Oversee and mentor Physical Therapy Assistants and, when required, Certified Nursing Assistants.
