Contract onsiteOverviewOur Award Winning Client, operating nationally and recognized for excellence, is seeking a skilled Physical Therapist to join their team. This position involves day-to-day therapeutic intervention for appropriate individuals in a rehab environment, adhering to the policies and procedures established by the facility, Medicare, private insurance, and our Award Winning Client. Duties And Responsibilities
Provide direct patient care as prescribed, including evaluation and treatment planning.
Maintain and ensure the accuracy of records regarding patient care for all patients on the personal caseload.
Correctly report billing information to the Director of Physical Therapy for all patients on the personal caseload.
Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and our Award Winning Client's policies and procedures.
Assist and assess transfers, gait, and balance as required by the functional mobility of patients, ranging from minimum to maximum levels of assistance.
Participate in the development and presentation of in-services for the facility as requested.
Supervise support personnel in immediate work areas as appropriate.
Participate in meetings regarding patient care as appropriate.
Assist with community education and public relations activities of the facility when appropriate.
Educate the patient’s family and staff on the specific treatment plan and therapy goals for the patient.
Coordinate and cooperate with other staff members in carrying out the established treatment plan.
Perform manual manipulation, including pushing and/or pulling of patient body parts and limbs.
Report and interpret treatment results.
Perform other duties as assigned.
Qualifications
Bachelor’s degree or higher in Physical Therapy.
Valid certification with the American Physical Therapy Association.
Licensed by the State of Arizona in the designated geographic area in his/her respective therapy profession.
Demonstrates the ability to perform the following tasks without posing a threat to the health or safety of him/herself or others and without risk to self, co-workers, caregivers, or patients: lift, turn, move, position, and transport the patient into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
Ability to perform sedentary tasks related to the job – approximately 10%.
Ability to perform physical tasks as listed – approximately 90%.
Demonstrates the ability to perform a minimal assist transfer, defined for this document as the ability to lift and control 50 pounds.
