We are now hiring for a full-time Title III B Program, Senior Services Case Manager to join our Team at Southeast! SALARY: Compensation is commensurate with years of related experience, position requirements, and candidate qualifications. The average salary for the position is $45,571.00.Essential Job Functions
  • Conducts thorough assessment of clients. Evaluates problem and service needs
  • Develops a written client service plan specifying goals to be achieved and services to be pursued.
  • Supplements service plans to meet client needs.
  • Provides, as appropriate, supportive help, advocacy and case management to individuals and families in coping with environment and interpersonal relationship issues.
  • Works collaboratively with community resources. Develops a knowledge of resources and helps clients to utilize them.
  • Works within contract, grant, and program requirements including meeting timelines and reporting requirements.
  • Reassess and monitors client situation and adequacy of service plan.
  • Collaborates and participates with other program staff in the planning and implementation of service plans.
  • Subsequent to consultation with supervisor, handles emergency and crisis situations within target population.
  • With supervisory approval, authorizes limited amounts of financial assistance to own clients.
  • May work with community groups.
  • Works effectively with diverse staff and service population.
  • Provides specific information about appropriate community resources that meet the immediate expressed need.
  • Provides assistance to older person, persons with disabilities, or their caregivers to identify their needs and to place them in contact with appropriate community resources or service providers.
  • Conducts formal and informal assessments of client, identifying problem areas, service needs, and capacity of the individual beyond the presenting problem followed by guidance and decision support.
  • Conducts follow up activities with clients and/or agency(ies) to determine whether services have been received and the identified needs met following the formal referral.
  • Assists clients in filling out applications, obtaining authorizations and follow ups with clients to ensure that services are accessed.
  • Conducts one-on-one and group outreach for the purpose of identifying potential clients and encouraging use of the agency’s services and benefits.
  • Maintains competencies in working with diverse populations to successfully engage clients in assessments, care planning, and referral and linkage.
  • Completes documentation/data entry on services rendered, attempted contacts, staffings, supervision, etc. in EHR, health population platforms, and/or funder required data bases in a timely manner.
  • Completes and/or runs reports specific to populations served to ensure tasks are completed in a timely manner.
  • Required to work beyond traditional work hours, including late nights and weekends, to meet the needs of the program.
Qualifications
  • Bachelor's Degree in Social Work or related field from an accredited college or university, required.
  • One year experience in social service provision to older persons preferred.
  • Computer competency sufficient to navigate between record keeping systems, required.
  • Working knowledge of technology and assistive devices related to caregiving.
  • Strong interpersonal and time management skills.
  • Some direct service experience preferred.
  • Requires mobility to work with a community based caseload.
  • Ability to maintain assigned community and home-based services.
  • Ability to work effectively with diverse individuals and groups.
  • Working knowledge of the Senior Health Assistance Program (SHAP) and MIPPA standards, the Illinois Department on Aging (IDoA) Adult Protective Services program and resources and programs available for older adults and people with disabilities are preferred.
  • Senior Health Insurance Program (SHIP) certification preferred.
  • Certification in the delivery of Information and Assistance preferred.
PHYSICAL DEMANDS
  • While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch.
  • The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing and viewing a computer terminal.
  • The employee may occasionally lift and/or move up to 50 pounds.
