Provides administrative support for Case Management Agency (CMA) and clients in ensuring the delivery of adequate case management services in meeting the agencys goal and required objective in keeping clients safe in their homes as long as possible.

Be the back up to the intake case manager in answering phone calls and pre-screening clients for intakes

Assist case managers with making 2 months follow-up calls as needed.

Follow-up with Not at Home and (No Answer) notifications from meal provider

Monitors/ follows-up, and updates CM and supervisors on NAH & No Answer calls.

Updates CM and supervisors regarding clients complaints and problems.

Audit data in PeerPlace for accuracy

Maintain statistics

Coordinates services for CMA

Update CM and supervisors on complaints and resolutions.

Updates Supervisor regarding any delays or problems

Assist with outreach

Assist with data entry and all other administrative task as assigned

Associate degree from an accredited institution preferred, High School Diploma and one year experience working in social service or office environment.
