This position is responsible for assessing, coordinating and teaching the individual with diabetes and/or
their support person to manage their disease. The nurse works closely with physicians, nursing staff and
Diabetes Center (DC) team to plan and provide quality care to diabetic patients. Manages all the nursing
aspects of diabetic patient care and works with DC team designing classes and clinics relating to Diabetes
Mellitus.
(35%)*Assesses inpatient and outpatient's educational needs, develops and instructs patients ranging in
age from toddler to 100 years and implements a plan of care for diabetes education. Documents
appropriately in the patient chart.
(35%)*Engages in inpatient and outpatient/family teaching (group & individual) as appropriate. Evaluates
the patient's responses to the teaching and adjusts the education style and plan to meet the needs of the
patient.
(15%)*Uses Staged Diabetes Management Decision Paths as Diabetes Center protocol for insulin
adjustment.
(15%)*Teaches new technology to patients, i.e. Continuous Glucose Monitoring, Insulin Pumps, Inject able
medications, Insulin delivery devices and blood glucose meters
OTHER DUTIES AND RESPONSIBILITIES:Develops and implements comprehensive diabetes selfmanagement programs and classes that are of interdisciplinary nature.
Participates in special projects such as set-up of professional educational conferences, community
education, support groups, detection programs and marketing endeavors.
Plans and implements continuing education and/or reviews education content as needed for MyMichigan
Health staff including inpatient nursing staff, providers and outpatient medical office staff.
Participates in Diabetes Center Staff, Program and Advisory meetings. Actively participates with physician
and health professionals in conducting patient group classes and clinics, including insulin pump clinic.
All other duties as assigned.
CDE - Cert Diabetes Educator
Preferred Equivalent Experience - Certified Diabetes Educator preferred, with
maintenance of certification with at least 15 hours of continuing education per
year. If no certification, passing of test within three years of hire is expected.
RN - Registered Nurse
Education: Associates Degree
EXPERIENCE, TRAINING AND SKILLS:Two (2) years medical/surgical nursing experience at hospital or
medical center required.
Interpersonal skills needed to communicate successfully with individuals and groups and to communicate ideas and concepts in a clear and understandable manner.
Provides for the age specific needs of the population served according to department standards and
policies/procedures as evidenced by observation, documentation and peer feedback for the following:
Neonate/Infant, Toddler, Child, Adolescent, Adult and Later Adult.
Ability to work independently to produce desired results.
