PER DIEM Nurse Practitioner in SNF and Retirement Communities

Job Description

MiraNova Health is seeking compassionate Nurse Practitioner to join us in providing exceptional medical care to patients in continuing care retirement communities. This role offers the opportunity to meaningfully impact the lives of older adults while maintaining significant flexibility.

MiraNova Health is a small women-led practice that was founded in 2020 to provide the highest quality, patient-centered medical care to residents in nursing facilities, assisted living facilities and retirement communities.

Our practice prioritizes ample time with patients so that we can understand the complexities of their medical conditions and values as we care for them. We are focused on helping each of our patients achieve the highest level of health and dignity during the last phase of their lives.

Responsibilities:

• See both medically complex and frail patients during rehabilitative stays and longterm residence in skilled nursing facilities and assisted living facilities.
• Expectation to see 8-10 patients daily.
• Interdisciplinary teamwork with therapy team, nurses, pharmacists, nutrition providers as well as specialists to optimize patient’s well-being with a multifaceted approach.

REQUIREMENTS INCLUDE:

Education: Master of Nursing Practice or Doctor of Nursing Practice (Required)

Experience: Experienced and new ARNP grads are encouraged to apply. Past nursing experience with at least 2 years as RN in hospital or nursing home setting (Preferred).

License/Certification: Certified Nurse Practitioner with WA state licensure and DEA license (Required)

Skills:

• Effective communication and interpersonal skills and clinical knowledge in the care of geriatric patients.
• Proven excellence working in an interdisciplinary team environment.
• Proficiency in documentation in an electronic medical record and ability to navigate computer-based software and programs. We use the web-based EMR Gehrimed.
