Psychiatric Social Worker Position SummaryThe Social Worker provides the full spectrum of social services to all patients, their families and significant others, who receive services. This includes psychosocial evaluation, discharge planning, group therapy and individual/family consultations. These services are provided by the social worker in collaboration with the interdisciplinary team. Major ResponsibilitiesJOB RESPONSIBILITIES The Social Worker’s primary responsibility is to facilitate the effective delivery of social services by developing and maintaining effective communications with patients, families, the multidisciplinary treatment team and referral resources. As a patient advocate, the Social Worker designs the discharge plan with the patient and/or care provider and promotes the best possible plan upon discharge including aftercare referrals. Clinical ResponsibilitiesThe Social Worker demonstrates clinical competency in behavioral health theories and practices through the implementation of psychosocial services to patients and their families, including the utilization of appropriate community resources. Through interviews with patients, families, significant others, and community providers the Social Worker completes psychosocial assessments on all assigned patients. The Social Worker provides a variety of therapeutic modalities including group therapy, consultation to patients and their families, individual therapy in accordance with the treatment plan, other interventions as appropriate. He/She collaborates with members of the multidisciplinary team during treatment planning and utilization review meetings, initiating and designing appropriate social services interventions. The Social Worker formulates a plan for the timely discharge of each patient, including identification and documentation of aftercare services as needed. This process includes: Locating resources forplacement (including out of home placement) making referrals, interfacing with community agencies and families, coordinating continuity of care as much as possible. Social Workers have a dual-role as patient advocate and mental health provider. The Social Worker interfaces with community providers on behalf of the patient, yet also prepares and presents the patient’s medical records for judicial review at Probable Cause Hearings. The Social Worker must balance his/her sensitivity to individual patient needs and preferences with social service standards for safe, appropriate discharge planning. COMMUNICATION The Social Worker is able to maintain effective professional communications in both verbal and written form with all department administrators, medical staff, employees, patients, families and supervisors. He/She assist with the accurate, legal, and timely completion of assessments, reports, forms, and documents. At times the Social Worker serves as a liaison between patients, staff, physicians, community entities and utilizes principles of quality customer service in all communications. INITIATIVE The Social Worker consistently shows the ability to work independently with maximum productivity. Detail-oriented and organized, the Social Worker demonstrates the ability to make decisions, problem-solve, utilize effective time-management skills and be self-directed in carrying out duties and responsibilities. The Social Worker consistently: coordinates tasks to achieve maximum productivity and efficiency; performs duties in an independent manner with little or no need for direct supervision; shows the ability to see what needs to be done without being told, responds promptly and positively when directed to perform a duty; doesn’t abuse sick days or personal days off; is available when coverage is needed; and keeps personnel File/certifications/education Status Current.QUALIFICATIONSMasters Degree in Social Work from an accredited college and at least a year of previous work experience in a hospital setting and/or at least a year of previous work experience in a setting with the psychiatric population. Eligible for licensure.
