We are currently looking for qualified candidates for the role of Director of Population Health & Quality Improvement . This position reports to the Chief Operations Officer (COO)  and is a Regular, Full Time, Exempt role with a starting yearly salary range of $108,400 to $119,700 with full benefits.
Primary Objective:

The Director of Population Health and Quality Improvement (DPHQI) will be primarily responsible for leading efforts to improve both population health outcomes and clinical quality, while enhancing satisfaction for patients and staff. This role focuses on developing and implementing strategies to manage the health of populations, reducing health disparities, and improving overall health outcomes. The DPHQI will oversee and support key systems vital to clinical quality and operational efficiency, including schedule utilization, patient wait times, and other performance metrics. In ensuring MNHC maintains Patient Centered Medical Home (PCMH) recognition, the DPHQI will lead system improvements in patient access, productivity, population-based quality improvement, and patient care management at both the team (provider/medical assistant) and population levels. Additionally, this position will oversee the ambulatory quality and data analytics teams, ensuring accountability for meeting metrics and reporting data to all required agencies (federal, state, and local) as well as internal Quality Improvement programs.
The DPHQI will work closely with the Director of Clinical Operations to design and implement comprehensive quality improvement initiatives, fostering a culture of excellence, innovation, and continuous improvement in healthcare delivery and patient outcomes.
Essential Functions/Responsibilities
Performance Management & Leadership Development:
  • Supervise assigned staff including recruitment, hiring, orientation, training, retention, performance improvement coaching, and performance evaluation, providing both technical oversight and high level project planning.
  • Lead the development and implementation of performance management and leadership development programs to foster a culture of continuous improvement and professional growth.
  • Chair the Quality Management Committee, providing strategic direction and oversight.
  • Provide leadership and oversight for primary care service delivery improvement initiatives and programs such as NCQA Patient Centered Medical Home, UDS, and HCAI.
  • Oversee MNHC’s participation in health plan Pay for Performance programs.
  • Coordinate preparation for external audits and site visits by health plans and governmental agencies.
  • Ensure timely follow-up on required corrective action plans and evaluate performance improvement activities.
  • Stay informed about relevant local, state, and federal laws impacting clinic operations.
  • Oversee internal reporting systems for clinical indicators and clinic operations metrics.

Population Health & Quality Management:
  • Develop and implement strategies to manage the health of populations, reducing health disparities, and improving overall health outcomes.
  • Guide the work MNHC’s community health workers/promotoras to ensure alignment with organizational objectives and successful implementation of initiatives.
  • Guide MNHC through the restructuring and alignment of Medi-Cal CalAIM services across the organization.
  • Utilize EHR and practice management functions and population health functionality to track, measure, and publicize population-based clinical and productivity goals.
  • Lead efforts to enhance population health management through data analysis, identifying trends, and implementing evidence-based interventions.
  • Ensure MNHC maintains Patient Centered Medical Home (PCMH) recognition and leads system improvements in patient access, productivity, and population-based quality improvement.
  • Oversee the review, implementation, and monitoring of MNHC’s Quality Management Plan.
  • Lead the adoption of new clinical and operations performance measures, including those originating from health plan pay-for-performance programs.
  • Ensure the quality of clinical operations, including schedule utilization, patient wait times, and other performance metrics.
  • Develop and oversee quality improvement initiatives, ensuring continuous improvement in healthcare delivery and patient outcomes.
  • Oversee the ambulatory quality and data analytics teams, ensuring accountability for meeting metrics and reporting data to required agencies and internal Quality Improvement programs.
  • Lead efforts to track and measure clinical and productivity goals, utilizing advanced knowledge of EHR practice management and population health registry functions.
Stakeholder Engagement:
  • Engage diverse stakeholders to develop and implement comprehensive quality improvement plans.
  • Represent MNHC in external quality improvement committees and collaborative initiatives.
  • Make recommendations and presentations to the Board of Directors, Executive Team, MNHC staff, and other external agencies, boards, commissions, and offices as needed.
  • Lead initiatives to build and maintain relationships with key stakeholders, fostering collaboration and support for quality improvement efforts.
  • Mobilize staff and teams, building momentum to achieve goals by communicating clearly and consistently, and investing time and energy to engage all stakeholders
Other:
  • Participate in Executive/Senior Management, Risk Management Committee, Clinical Leadership, Managers and Supervisors/Leadership Circle, and Department meetings.
  • Perform other duties as assigned
Qualifications
We are looking for a candidate who has demonstrated success in the following areas:
  • Quality Improvement and Data Analysis: Establishing and maintaining high standards, synthesizing data into actionable insights, and independently owning complex analyses to drive improvements.
  • Population Health: Managing multiple projects and responsibilities across various sites, ideally in an outpatient clinic model, to enhance health outcomes.
  • Inspirational Leadership and Team Engagement: Cultivating a culture of excellence through authenticity and humility, empowering and coaching individuals to reach their full potential, and engaging diverse stakeholders to develop and implement goals.
  • Effective Communication and Customer Service: Establishing trust, demonstrating poise and perspective in communication, and working towards solutions with teammates at all levels while meeting the needs of colleagues and patients with dignity and respect.
Minimum Qualifications:
  • Two years’ experience in ambulatory care clinic or hospital experience; community clinic experience preferred.
  • Three years increasingly responsible management and supervisory experience.
  • Excellent verbal and written communication skills.
  • Ability to work collaboratively with multiple levels of staff.
  • Experience with project management tools and methods.
  • Knowledge of and experience with quality and process improvement methodologies.
  • Knowledge of Microsoft Office applications (Word, Excel, PowerPoint)
  • Familiarity with Electronic Health Record systems.
  • Ability to work professionally and ethically within multi-cultural settings, including ability to maintain confidentiality and privacy of persons, documents, data, and communications
Preferred Qualifications:
  • Master’s Degree in Public Health, Health Services Field or Business Administration
