The Admissions Director is responsible for timely and accurate responses to referrals to bring patients into the building. The director needs to ensure that all residents that are accepted are appropriately placed within the facility and staff is appropriately prepared to care for the admission. The Admissions Director must lead by example by promoting positive morale within their department and throughout the facility by smiling and greeting residents, families, and all employees.

• Assess referrals through electronic referral systems, as well as phone and faxed referrals to determine appropriateness of admission.
• Communicate with referral sources and Liaisons on admissions and transfers to the facility.
• Communicate with clinical supervisors regarding admissions preparations and any concerns, such as special circumstances, equipment needs, orders, skilled needs, and caregiver or family member concerns.
• Obtain accurate information from referral sources including discharge planners, care givers, physicians, and other hospital staff to prepare for the patient’s admission to the facility.
• Set up and coordinate any readmissions to the facility, including keeping the facility informed on the condition of the hospitalized patient, contacting Liaisons and discharge planners at the hospital regarding the readmission, and obtaining updated clinical information.
• Serves as daily contact with patients, families, medical personnel, and other potential referral sources.
• Coordinates with Case Management on any Managed Care referrals for admission to obtain authorization and more.

• Bachelor’s degree from an accredited college or university in marketing, business or related field preferred.
• Two years successful marketing experience in the health field preferably in long-term care and/or subacute rehab highly preferred.
