Responsibilities:
The Wound Specialist will work 20 hours per week and will work with the interdisciplinary team to maximize the clinical outcomes of our complex wound cases.
The Wound Specialist will provide guidance to physician and non-physician providers on evidence based recommendations for wound management.
They will collaborate with the nursing team and may assess patients in their homes as needed.
Based on patient assessments, the Wound Specialist will develop a plan of care, specific to the management of the patient's wound, to meet their goals.
Collaborates with staff and makes recommendations regarding wound and ostomy care and supplies.
Establishes competencies for clinicians regarding wound care management while collaborating with other members of the education and clinical team on joint initiatives.
Collaborates with members of the interdisciplinary team to encompass all aspects of care to promote wound healing.
Qualifications:
Current New York State RN license.
Bachelor of Science degree in Nursing.
Master's Degree preferred.
Two years of experience in as a Registered Nurse.
Experience working in an acute, long term care facility or a Certified Home Health Agency.
WOCN, WCC, CWCN, CWOCN OR CWS Certification are required.
