Position Title: Home Health Physical Therapist

Location: Central Ohio

Employment Type: Part-Time

Schedule: Day Shift, Monday - Friday

Job Summary: MLee Healthcare is seeking a dedicated Home Health Physical Therapist to join our client's top-tier healthcare team. In this role, you will provide comprehensive physical therapy services to patients in their homes, ensuring the highest level of care and service excellence.

Key Responsibilities:

• Perform patient evaluations to determine physical therapy needs.
• Develop and implement individualized treatment plans.
• Conduct reassessments and adjust treatment plans as necessary.
• Manage OASIS SOCs (Start of Care), ensuring accurate documentation and compliance.
• Provide discharge planning and follow-up care as needed.
• Collaborate with physical therapist assistants to carry out prescribed treatments.
• Maintain clear and detailed patient records.
• Communicate effectively with patients, families, and healthcare team members.
• Educate patients and caregivers on home exercise programs and therapeutic techniques.

Qualifications:

• Licensed Physical Therapist in the state of Ohio.
• Previous experience in home health care preferred.
• Strong understanding of OASIS documentation requirements.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Valid driver’s license and reliable transportation.

Education:

• Bachelor's (Preferred)

License/Certification:

• Physical Therapy License (Preferred)
