The physical therapist willaccurately assess differentially diagnose and identify deficits ina patients ability to move and perform functional activities intheir daily lives.

They develop an individualized treatment planwith patientcentered goals to reduce pain restore function andprevent disability and work with the individual to prevent the lossof mobility.

Essential Functions andResponsibilities :

• Performs patientevaluations through the process of obtaining history performingrelevant systems reviews and selecting and administering specifictests and measures to obtain data.
• Establishesan individualized plan of care and selects treatment techniquesthat address the specific impairments identified in the evaluationand following physician orders.
• Conductsongoing reassessment of the patient and their response totreatment.
• Initiates and manages the dischargeplan beginning at the first visit and continuing throughouttreatment.
• Supervises and delegates toancillary staff and students consistent with state laws anddepartment policy.
• Completes legible accurateand clear documentation within the established timeframe identifiedin the department policy and procedure and following accreditationrequirements and insurance regulations.
• Communicates with the patient family caregiverssignificant others and members of the health care team to promotemaximum benefit of care utilizing AIDET.
• Participates in professional growth anddevelopment through participation in educational programs includinginservice meetings and workshops and by reading current literatureconcerning the scope of practice.
• Activelyparticipates in quality improvement activities.
• Provides an environment conducive to safety for patientsvisitors and staff. Complies with appropriate and approved safetystandards.
• Functions within the scope ofpractice as defined by the states practice act.
• Performs other related duties as required anddirected.
