Shift Type/Shift Times/Hours/Day: Days, 7:30 AM - 4:00 PM, 8
Shift Notes: 7:30A - 4:00P (8 Hour Days) / 5 days one week, 4 days the next week / Every Other or Every Third Weekend / No Call (32 Hour Guarantee)
Weekend Requirements: Every Other Weekend or Every Third Weekend

Dress Code: Business casual. No jeans. No scrubsThe Social Worker is responsible for creating outcomes for the patient and family by managing complex psychosocial and economic co-morbidities. Provides education and consultation regarding resources, collaborates to develop a discharge plan, and facilitates and monitors its
implementation. Assists patient/family in adjusting to and coping with illness, diagnosis, treatment options, and prognosis. Through advanced practice skills mobilize resources to reduce risk and serve as a liaison between the hospital and the community. In addition, offers crisis interventions to patients/families. Strives for superior performance by consistently providing a product or service to leadership and staff that is recognized as ultimately contributing to the patient and family experience. As a customer-focused Memorial Healthcare team member, he/she must be accessible, flexible, knowledgeable, and responsive.

Submission Requirements
- 2 years of experience - Required
- Master's degree in social work from an accredited college or university - Required
- LMSW - Required
- Mental Health Experience running psychotherapy groups - Required
- At least two years of experience in an inpatient or outpatient facility - preferred
- Clinical grief counseling experience - preferred
- Covid Vaccine - Required
- BLS (AHA)
- Michigan
- Active at the time of submission!
