Implements the agency program of fulltimeprofessional nursing service to patients in their homes under thesupervision of the Clinical Services Supervisor following agencystandards and policies.

Essential Functions andResponsibilities :

1.Initiates and provides medically prescribed skilled nursing careand / or health counseling that contributes to the treatment of themedical condition achievement of the rehabilitation potentialand / or reduction of pain and other symptoms.

2.Make the initial visit to assess the patients physical andemotional status needs and resources through observation interviewand analysis of records.

3. Prepares the nursingplan of care by the physicians plan of treatment based on problemsidentified in the nursing assessment and provides patient teachingusing the Care Plan / Standard of Care.
