We are seeking an experienced Home Health Care Registered Nurse (RN) to Join our Team…As the Case Manager (RN) with Altruist Home Health Care the primary functions include: skilled nursing care for clients of all ages in their place of residence, coordination of care with the interdisciplinary team to include the patient/family and referring agency; assumes the responsibility for coordination of care. The RN Case Manager is a position within our Clinical department and will be responsible for reporting to the Director of Clinical Services.

Job Qualifications

Education

· Graduate of an accredited Diploma

· Associate or Baccalaureate School of Nursing (Required)

Licensure

· Current Texas State or Compact license as a Registered Nurse

· Current Texas Driver's License

Experience:The ideal candidate will have three to five years of experience as a Home Health Care Registered Nurse or Registered Nurse in a clinical care setting. (Required)

Skills:

• Nursing skills as defined as generally accepted standards of practice.
• Good interpersonal skills.
• Proof of current CPR, negative TB screen, and Hepatitis profile.
• Must be compassionate about home health care.
• Must be able to see patients in the surrounding metropolitan area.
• Spanish speaking is a plus!

Salary: $75,000.00 - $77,500.00 per year
