JOB SUMMARY
Responsible for the efficient patient flow throughout Mon Health Medical Center and Vandalia North taking into consideration the patient's physical condition, the referring physician's needs and the institutions resources to ensure timely and appropriate discharges.

Experience/Physical Requirements :
Light Work: Frequent lifting and/or carrying of objects weighing up to 10 lbs. with a maximum lifting of 25 lbs. Minimum requirement: ASN with 3 years experience in ED, ICU, or House Supervisor. Preferred BSN with 3-5 years experience in ED, ICU, or House Supervisor. Effective interpersonal communication skills in order to effectively collaborate with physicians, nursing staff, patients and families. Always demonstrates respect and professionalism.

JOB SPECIFIC RESPONSIBILITIES
Service Excellence Demonstrates the ability to engage internal and external providers, nursing staff and leaders within Vandalia Health, by consistently upholding Mon Health’s Standards of Behavior during every interaction. Works diligently to meet the ‘Yes Always’ philosophy and goal of being the easier place to move patients with the system. Interprofessional Relationship and Teamwork Participates in multidisciplinary team rounds (MDRs) on all nursing units helping to facilitate safe and effective patient discharge. Develops relationship with physicians to work on discharge needs of patients. Working within MDR framework, identify patients who are candidate for discharge with nursing units and assign accountability in completing necessary tasks. Daily follow up with nursing units for feedback of plan adherence. Ensures collaboration occurs between nursing units, social workers, and case management. Communication Participates in Vandalia Health North daily bed meeting to receive updates on hospital(s) census, staffing and patient acuity, anticipating unit needs, and surgery needs, external referrals, and pending/confirmed discharges. Works collaboratively with the Vandalia Health Transfer Center, monitors patient tracks via Awarix and ED bed board to ensure efficient process and flow of patients. Serves as contact to Transfer Center when patient deferral is under consideration to facilitate communication with physicians, medical directors, and nurse managers. Works with all clinical support departments to optimize inpatient Turn around times (TAT) processes. Works with all non-clinical support departments to optimize inpatient TAT processes. Evaluates communication processes occurring between transfer center and nursing units to streamline placement of patients. Stewardship/Quality Identifies standardization opportunities for patient placement, transfers, and timely discharge. Serves on organizational Patient Flow Committee and provides outcomes of related projects. Integrates patient flow initiatives with nursing unit transforming care together project work. Identifies and resolves barriers for effective and efficient patient disposition. Evaluates transfers between Vandalia North hospitals. Ensures appropriate patient placement and helps to minimize mis-assignment and movement between units.
