Monitor My Health, Inc. is Hiring a Program Coordinator - Diabetes Prevention Program Near Hamden, CT

We are looking for someone who has extensive experience overseeing AND/OR delivering the Diabetes Prevention Program (DPP). To serve 150 patients with diabetes across CT, your role will be to help us with the outreach, coordination, logistics, supplies, materials, and data assessments. If you are certified to deliver DPP - you will instruct the classes as well. If you are not certified to deliver DPP- you will help us to manage the team of DPP Coaches. We estimate this job to take 15-20 hours per week. There is a potential for Full-Time employment in the future.

Responsibilities:

  • Scheduling: Create and maintain schedules to ensure they are up-to-date. Monitor schedules for adherence and make necessary adjustments.
  • Team Assembly: Facilitate the formation and coordination of program teams. Ensure the appropriate skills and resources are allocated to each team.
  • Communication: Conduct program, logistical, technical, and supplies support to team members. Facilitate interactions among team members.
  • Site Visits: Conduct site visits to oversee and support program activities. Ensure that program objectives are being met on-site.
  • Attendance and Record Monitoring: Monitor attendance of program participants and monitor and maintain other relevant program records. Track and report on key performance indicators as needed.
  • Incentive Program Implementation: Develop and implement incentive programs to motivate and recognize team members. Monitor the effectiveness of incentive programs and make adjustments as necessary.
  • Assist with Reports: Support the preparation of reports, including surveys and other documentation. Compile and analyze data for reporting purposes.
  • Stakeholder Liaison: Serve as a point of contact between the program and external stakeholders. Communicate with partners, sponsors, and other relevant entities.
  • Training and Development: Coordinate training sessions for team members to enhance skills and knowledge. Facilitate professional development opportunities.

Qualifications
– Education and/or experience in program coordination/management.
– Education in, and/or experience with health promotion programs and nutrition (preferred: Diabetes Prevention Program).
– Supervisory abilities- Proficient in various computer applications, including spreadsheets
– Sufficient experience to carry out the duties of this position
– Ability to relate with warmth and effectiveness to the patients and to the providers of Monitor My Health

Job Type: Part-time

Pay: $30.00 - $40.00 per hour

Expected hours: 15 per week
