Quality Coordinator

  • Temp only 20 weeks
  • Monday to Friday 8am 5 pm
  • Location : San Ysidro CA92173 (onsite)
  • Will bemaking outbound calls constantly on the phone doing outreachhandling excel spreadsheets scheduling appointments repetitive innature
  • Must have EPICexperience
  • Must beBilingual (English / Spanish)
  • Health related call center experience is a plus
  • Must have minimum 2 yearsexperience

Position Summary :

Under the supervision of the Quality Manager assist in allactivities related to quality management program which includes : assisting with organizational adherence to federal state and otherregulatory requirements;

monitoring and maintaining compliance withNCQA PCMH recognition standards; TJC ambulatory accreditationsstandards; and facilitating a state of continuous clinic auditreadiness.

Essential Functions of theJob :

  • Track key performanceindicators related to regulatory clinic compliance audits surveysand studies by health plans federal and state agencies.
  • Track and support as directed by the Quality Managercontinuous audit readiness activities.
  • Supportcare team training activities to improve clinical qualityperformance and adherence to regulatory health plan and otherprogram participation requirements.
  • Coordinatepeer review activities by maintaining an annual calendar preparingrecords for review and collecting data for review.
  • Provider administrative support for quality improvementstudies addressing areas of opportunities. Maintain and updatedepartmental project work plans as needed.
  • Participate in departmental PCMH and CQI meeting foralignment and tracking of active projects.
  • Facilitate project milestones by coordinating followupactivities internally and cross departmentally.
  • Provides support in capturing and updating policiesprocedures practice guidelines and clinic protocols that impactclinical quality.
  • Keep abreast of PCMH and TJCprogram requirements and updates.
  • Assistgathering data for quality related reports for regulatorysubmission and as requested by the board Executive and Departmentleadership.
  • Adheres to departments attendanceand punctuality policies and practices.

ADDITIONAL DUTIES ANDRESPONSIBILITIES :

  • Performother duties as assigned.
  • Enhance professionalgrowth and development through participation in educationalprograms current literature inservice meetings andworkshops.
  • Attend meetings as required andparticipate in committees as directed.

JOBREQUIREMENTS :

EducationRequired (Minimum level of education) :

High School Diploma

Valid California drivers license withappropriate insurance.

Experience Required (Minimum level ofexperience) :

  • Two yearsexperience in healthcare or a closely related field; familiaritywith the PCMH model of care the continuum of care transitions ofcare and quality improvement processes.
  • Experience in chart review and medical recorddocumentation.

Verbal and Written Skills Required to Perform theJob : Excellent oral and written communication skillsrequired.

Technical Knowledge andSkills Required to Perform the Job : Proficient inMicrosoft Excel Power Point and Word and attention todetail.

Equipment Used : Networked personal desktop document center and telephone.

Working Conditions and PhysicalRequirements : Prolonged periods of sitting and standingdriving within the country to travel between clinic locations andto occasional community partner meetings.

May be required to workevenings and / or weekend.
