Mountain Home is Hiring a Case Manager Near Missoula, MT

Mountain Home seeks two (2) Case Managers (CM) to help improve outcomes, experiences, and value for the families served.
The CM provides case management to young mothers, adolescents, and children, linking them to needed medical, social, educational, vocational, and recreational services and providing advocacy to support and increase their independence in the community.
The ideal candidate is familiar with social services in the Missoula community, has knowledge of mental illness and treatment, has two or more years of experience providing case management or similar services, and has or is working toward a bachelor's degree in social work, human services, or a related field.
Status: Non-Exempt, Non-Shift, Full-time, 36 - 40 hours/week
Pay Range: $19.50 - $24.50, based on years of experience, education, and training

Schedule: Monday - Friday with some evenings (no later than 8:00pm) required
Location: Hybrid- 80% of work will be performed in the community, client homes, or office, 20% performed from home
Travel: This position requires frequent local travel, approximately 30% of weekly time using company vehicles
Reports To: Lead Case Manager
Essential Functions
  • Meeting weekly for case management with clients and maintaining individual client files
  • Building rapport with clients while maintaining professional boundaries at all times
  • Providing services primarily in the community or clients' homes
  • Providing staff with client history/special requirements and hosting team meetings
  • Making referrals to other services such as mental health services, parenting, etc.
  • Communicating with clients' therapists for care coordination and treatment planning
  • Providing transportation and serving as an advocate for clients at appointments
  • Completing and overseeing the discharge process for all assigned clients
  • Documenting dates, times, and progress notes for all meetings in an online database
  • Meeting and maintaining billing hours expectations and documentation timelines
  • Attending meetings and training as required and meeting regularly with their supervisor
  • Other appropriate and job-related tasks as assigned
KEY PERFORMANCE INDICATORS
  • 100% of documentation is recorded accurately within required timelines
  • Goal billable hours are met at least 90% of time
  • Treatment plans are completed to standard within provided timeline
  • Progress on treatment plans is tracked and documented to standard and case consultation is sought regularly to best facilitate client progress
  • Discharge process is completed following policy and department timelines
MEASURES OF ACCOUNTABILITY:
  • 50% of time allocated to case management sessions with clients
  • 20% of time allocated to documentation of case management with clients
  • 10% of time allocated for client coordination and communication
  • 10% of time allocated to supervision, meetings, and collaboration
  • 10% of time allocated to continuous professional development
